Swedish Transition Effects Project Supporting Teenagers with chrONic mEdical conditionS
In November 2018, the rationale and design of the Stepstones-CHD project was presented by Philip Moons at the 7th Conference of the German Society for Transition Medicine in Münster, Germany.
On march 7th 2018 we had our fourth annual meeting with our Advisory Board. Veronica Danko (patient perspective), Malene Jensen (parent perspective), Peter Nordkvist (patient representative), Kristina Berg-Kelly (specialist in adolescent medicine), Inger Ekman och Karl Swedberg (person-centered care) met the research group and discussed the project. The research group presented how far have we come.
The abstract entitle “Using intervention mapping in developing complex nursing interventions-the STEPSTONES transition program for adolescents with chronic conditions” has been accepted in the European Conference in Nursing and Nursing Research (July 4-5, 2017) in Malmö, Sweden. Philip Moons and Ewa-Lena Bratt were invited speakers during “Barn vecka” in Västerås, 23-27 April.
STEPSTONES DIAB presented their upcoming work in a well visited workshop on “Transition from pediatric to adult care”. This workshop was arranged by Anna Lena Brorsson and Carina Sparud Lundin in cooperation with nurse specialists in diabetes care Ingela Lavin and Erika Persson.
