Swedish Transition Effects Project Supporting Teenagers with chrONic mEdical conditionS
On March 3rd 2016 we met with an International Panel of Experts to discuss different experiences and aspects of healthcare in the transition of young people with long-term illnesses. The meeting was held in Copenhagen.

STEPSTONES DIAB presented their upcoming work in a well visited workshop on “Transition from pediatric to adult care”. This workshop was arranged by Anna Lena Brorsson and Carina Sparud Lundin in cooperation with nurse specialists in diabetes care Ingela Lavin and Erika Persson.
During 11-12th and 18-19th of April the transition coordinators were invited for training and education.
