Swedish Transition Effects Project Supporting Teenagers with chrONic mEdical conditionS
The 21-23 September the was arranged by Children´s Heart Centre in Lund, Sweden. Ewa-Lena Bratt represented Stepstones with two oral presentations and one poster presentation.
STEPSTONES DIAB presented their upcoming work in a well visited workshop on “Transition from pediatric to adult care”. This workshop was arranged by Anna Lena Brorsson and Carina Sparud Lundin in cooperation with nurse specialists in diabetes care Ingela Lavin and Erika Persson.
We had the first meeting with the Advisory Board on February 25th, 2016 at the Institute of Health and Care Sciences, Sahlgrenska Academy, University of Gothenburg.
On the 14th of November the first adolescent day was held för participants in STEPSTONES (the picture shows the preparation before the meeting). The meeting was held at Karolinska University Hospital for the participants reciding in Stockholm and amongst others, young persons with a congenital heart defect along with staff from the adult cardiology clinic…
